QUALIFICATIONS

Education

  • Required: Medical Degree from an accredited institution.
  • Required: Residency training in Physical Medicine & Rehabilitation (PM&R).
  • Preferred: Fellowship training in Interventional Pain Management or Spine.

Licenses & Certifications

  • Active Pennsylvania Medical License (or eligibility).
  • Board Certified or Board Eligible in PM&R (and Pain Medicine, if fellowship-trained).

Experience

Minimum of three (3) years of experience in a related field (including residency or fellowship training).
