Clerical Assistant – Inpatient Care Management

Part-time, 3 days per week 8-4:30, weekend and holiday rotation, 48 hours bi-weekly

As a Clerical Assistant in Inpatient Care Management, you’ll support the team in facilitating safe, timely transitions for hospitalized patients. From scheduling follow-up appointments to arranging transportation and delivering regulatory notices, you’ll play a key role in ensuring continuity of care and compliance with quality standards.

This role is ideal for a detail-oriented professional who thrives in a collaborative environment and values both administrative precision and compassionate patient interaction.

Key

Responsibilities

  • Perform general office and secretarial duties to support care coordination and discharge planning
  • Deliver the Important Message to Medicare beneficiaries and assist with the appeal process when needed
  • Schedule post-discharge follow-up appointments to meet quality performance metrics
  • Meet with patients to educate them about appointments and identify barriers to attendance
  • Arrange patient transportation as indicated
  • Assist in the development and implementation of departmental projects and performance improvement initiatives (Katas)
  • Maintain accurate documentation and support efficient department operations

Minimum

Qualifications

Education

  • High school diploma or equivalent required
  • Associate degree preferred

Experience

  • Previous clerical experience in a healthcare environment preferred
  • Experience with Microsoft Office, fax machines, and electronic medical records preferred

Knowledge, Skills, and Abilities

  • Effective communication and computer skills required
  • Basic understanding of medical terminology required
  • Knowledge of community resources preferred
  • Ability to prioritize tasks and manage multiple responsibilities in a fast-paced environment
