About The Role
Clerical Assistant – Inpatient Care Management
Part-time, 3 days per week 8-4:30, weekend and holiday rotation, 48 hours bi-weekly

As a Clerical Assistant in Inpatient Care Management, you’ll support the team in facilitating safe, timely transitions for hospitalized patients. From scheduling follow-up appointments to arranging transportation and delivering regulatory notices, you’ll play a key role in ensuring continuity of care and compliance with quality standards.

Key
Responsibilities
- Perform general office and secretarial duties to support care coordination and discharge planning
- Deliver the Important Message to Medicare beneficiaries and assist with the appeal process when needed
- Schedule post-discharge follow-up appointments to meet quality performance metrics
- Meet with patients to educate them about appointments and identify barriers to attendance
- Arrange patient transportation as indicated
- Assist in the development and implementation of departmental projects and performance improvement initiatives (Katas)
- Maintain accurate documentation and support efficient department operations

Minimum
Qualifications
Education
- High school diploma or equivalent required
- Associate degree preferred
Experience
- Previous clerical experience in a healthcare environment preferred
- Experience with Microsoft Office, fax machines, and electronic medical records preferred

Knowledge, Skills, and Abilities
- Effective communication and computer skills required
- Basic understanding of medical terminology required
- Knowledge of community resources preferred
- Ability to prioritize tasks and manage multiple responsibilities in a fast-paced environment
