Non-Medical Case Manager

Key responsibilities and day to day responsibilities of this position: To provide HIV non-medical case management to make sure Ryan White Part B clients enter and remain in HIV medical care and have access to and are adherent on their HIV medications. Provide case management in accordance with the Case Management Service Standards for the Utah Ryan White Part B Program Perform client intake Assist in application and re-certification for the Part B Program Assess needs utilizing assessments as provided by the Program Develop care plan Coordinate care and benefits Provide and track referrals Provide internal and external resources Perform follow-up as needed and indicated by acuity and assessment scores.
