**Certified Nursing Assistant (CNA)**

Join a compassionate and supportive hospice team dedicated to making a meaningful difference in the lives of patients and families. The Care Team Hospice is seeking a **part-time CNA** to provide personal care and comfort-focused support to hospice patients in their homes and in facility settings, working alongside our interdisciplinary team to ensure dignity, compassion, and quality of life throughout the hospice journey.

**Schedule, Hours & Territory**

• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM. This is a part-time role.
• This role will cover Tawas, MI, and territories within an approximate one-hour radius of the market.

**Key Responsibilities**

• Provide personal care, hygiene, and comfort measures
• Observe and report changes in patient condition
• Support patients and families with compassion and respect
• Document care provided accurately and timely in the EMR HCHB (Homecare Homebase)

## Qualifications
**Qualifications**

• Active CNA certification
• Hospice aide experience preferred
• Experience using HCHB (Homecare Homebase) preferred

**Field Role Requirements**

• Active driver’s license
• Ability to pass a criminal background check
• Ability to pass a Motor Vehicle Record (MVR) check
• Current TB testing in accordance with agency policy
• Current CPR certification as required for the role
