Join a compassionate and supportive hospice team dedicated to making a meaningful difference in the lives of patients and families. The Care Team Hospice is seeking a **part-time Social Worker (MSW)** to provide emotional support, counseling, advocacy, and community resource coordination for hospice patients and their loved ones while collaborating closely with the interdisciplinary team to ensure compassionate, patient-centered end-of-life care. This role could turn into a full-time opportunity as our census continues to rise.

**Schedule, Hours & Territory**

• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
• Patients may be located in Macomb, Oakland, Wayne, Livingston, and Washtenaw Counties (mileage reimbursement is applicable)

**Key Responsibilities**

• Conduct psychosocial assessments
• Provide counseling and emotional support to patients and families
• Assist with advanced care planning and coordination of community resources
• Participate in interdisciplinary team meetings
• Document services accurately and timely in the EMR HCHB (Homecare Homebase)

## Qualifications

• Master’s degree in Social Work preferred
• Bachelor’s degree in Social Work accepted with required oversight
• Hospice experience preferred
• Experience using HCHB (Homecare Homebase) preferred

**Field Role Requirements**

• Active driver’s license
• Ability to pass a criminal background check
• Ability to pass a Motor Vehicle Record (MVR) check
• Current TB testing in accordance with agency policy
• Current CPR certification as required for the role
