Join The Care Team as a part-time Hospice LPN and make a meaningful impact by providing compassionate, hands-on care and support to patients and families during life’s most important moments. We are seeking a dedicated Licensed Practical Nurse to deliver high-quality patient care, support the interdisciplinary team, and help ensure comfort, dignity, and exceptional end-of-life care in home and facility settings.

**Schedule, Hours, & Territory**

• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
• Greater Indianapolis, IN area

**Key Responsibilities**

• Provide nursing care as delegated by the RN
• Monitor patient status and report changes
• Support comfort and symptom management
• Document care provided in the EMR
• Collaborate with the interdisciplinary team

## Qualifications

• Active Licensed Practical Nurse (LPN) license
• Hospice or long-term care experience preferred

**Field Role Requirements**

• Active driver’s license
• Ability to pass a criminal background check
• Ability to pass a Motor Vehicle Record (MVR) check
• Current TB testing in accordance with agency policy
• Current CPR certification as required for the role
