**Position:** Referral Coordinator-Open Access
**Position Type:** Full-Time
**Location:** 515 28 3/4 Rd, Grand Junction, Colorado – Building A / Supporting all locations virtually

**Qualification:**
Required: Baccalaureate degree in social work, psychology, or closely related field required. Experience in conducting clinical interviews and triage assessments, family case management, and community systems navigation desired. Experience working with a multi-disciplinary team. Must be a self-starter, self-motivator and possess exceptional time management, presentation and documentation skills, computer proficiency. Valid Colorado driver’s license and access to transportation during working hours required.
**Preferred**: Experience working with children/families as well as school and judicial systems preferred. Spanish language skills. CAC training. Experience working with individuals experiencing co-occurring disorders.

**Responsibilities:**
The Referral Coordinator – Open Access assists with coordinating Open Access internally and manages documentation for external referrals. External stakeholders include, but are not limited to, DHS, DOC, Probation, Parole, psychiatric facilities, treatment facilities, and hospital emergency rooms to connect individuals with services within Health Solutions West. Under the supervision of the Open Access clinical supervisor, the Referral Coordinator
**Referral Coordinator – Open Access will:**
- Function as a liaison between Health Solutions external stakeholders including but not limited to DHS, Probation, Parole, DOC, psychiatric facilities, treatment facilities, and hospital emergency rooms to ensure community members are connected with services within Health Solutions West.
- Maintain and monitor outpatient referral inbox funnels as assigned.
- Perform screenings (SDoH, IDS, Out-of-State Offender, suicidality and safety planning, etc.) and coordinate completion of Open Access Enrollment.
- Connect clients with community resources to address social determinants of health needs directly, and through coordination with intake assessment clinicians for ongoing outpatient care.
- Support drop-in assessment clients with immediate needs, triage, and enrollment navigation.
- Provide case management and care coordination services to individuals initiating care at Health Solutions West that have not yet engaged with their outpatient team.
- Manage client charts through documentation of care, and ensuring timely upload of registration documents and signed consents conducted personally and by the Support Services team in compliance with agency standards and OA workflows.
- Maintain competency and proficiency with the agency’s Electronic Health Record.
- Coordinate with Health Solutions West internal programs to support continuity of care and timely linkage to outpatient services.
- Work with some of our most difficult to engage individuals who are experiencing barriers to engagement in outpatient or higher level of care services to participate in care.
- Support Open Access referrals and enrollment excellence by coordinating with department leadership to promote policies and procedures that enhance Open Access operations.
- Identify systems issues causing disruption to organizational goals and client care and develop, initiate, and facilitate solutions for identified systems issues.
- Perform other job-related duties assigned by the supervisor or his/her designee
**Physical requirements include the following:**
- Must be able to remain in a stationary position at least 50% of the time.
- Frequently communicates with physicians about medical notes, must be able to exchange accurate information.
- Constantly operates a computer and other office productivity machinery, such as a calculator, copy machine, and computer printer.
- Occasionally lifts and carries items up to 20 pounds.
- Occasional exposure to communicable diseases and bodily fluids.
