## Description
**Essential Functions**

Patient Observation and Reporting

• Observes and reports changes in patient condition, behavior, mobility, appetite, skin integrity, or comfort level
• Monitors and documents vital signs, as needed
• Communicates patient concerns promptly to the supervising nurse or provider
• Reports on safety concerns and potential risks

Patient and Caregiver Support

• Provides emotional support and companionship to patients and families
• Reinforces education provided by the nursing and clinical team
• Assists caregivers with understanding basic comfort measures and safety practices
• Supports patients and families during difficult transitions related to serious illness

Care Coordination Support

• Schedules patient appointments, follow-up visits, and assists with appointment reminders
• Supports communication between patients, caregivers, and the clinical team

Nursing Assistant – Primary and Palliative Care

• Participates in interdisciplinary care discussions as appropriate
• Coordinates communication between patients, caregivers, providers, specialists, pharmacies, and community resources
• Interdisciplinary Team
• Attends and participates in interdisciplinary team conferences and special care conferences.
• Assumes responsibility for establishing and maintaining effective working relationships with team members.
• Can identify specific patient/caregivers needs and make referral to appropriate team members.
• Documentation
• Ensures accurate, complete, succinct, and timely clinical documentation in accordance with VIA Health Partners’ guidelines and the requirements of licensure and certification standards.

Professional and Personal Accountability

• Demonstrates competency in knowledge and skills.
• Ensures that VIA professional reputation is maintained and projected.
• Assumes responsibility for professional development and staying abreast of current trends in the healthcare field. Incorporates new information and methods into practice.
• Maintains current professional licensure, CPR certification and CEU requirements.
• Maintains productive and professional relationships with external agencies and healthcare providers.
• Exhibits time management skills. Able to plan workday while remaining able to respond appropriately to crisis.
• May perform other duties as required.

## Qualifications

Qualification Requirements

• High School diploma preferred, plus training/certification in related field required.
• Certified Nursing Assistant; maintains current listing status required.
• NA – must be in good standing with NC or SC Department of Health and Human Services
• Current CPR certification required

Minimum Qualifications

Minimum 1 years’ experience preferred. Experience in palliative care, primary care, hospice, home health, geriatrics, oncology, long-term care, and/or in providing care to patients with advanced illness preferred.

Knowledge of comfort-focused care and symptom management support and familiarity with home-based care preferred.
