**As a Physical Therapist, you will:**

• Evaluate patients referred by a physician, focusing on diagnosis-specific assessments.
• Maintain accurate and timely documentation, including daily time and mileage records, evaluations, and progress notes, in accordance with agency policies.
• Schedule patients for treatment as needed.
• Collaborate with other healthcare providers and disciplines to ensure cohesive patient care.
• Communicate with the physician and VNA staff regarding patient’s treatment progress and status.

**To qualify for a Physical Therapist position with us:**

• Education: Bachelor’s degree in Physical Therapy from an accredited school.
• Licensure: Must be licensed to practice as a Physical Therapist in the State of Indiana.
• Experience: At least two years of experience required; geriatric or home health experience preferred.
• Additional Requirements:
• Valid driver’s license and automobile insurance
• Successful completion of a criminal background check, post-offer physical exam, and drug screening
• Flexibility to travel as needed
