## Position Summary

As a Home Health LPN, you will work closely with patients, families, and the interdisciplinary care team to deliver skilled nursing services and support individualized care plans. This float position **serves patients throughout Central District and North King County.**

## Responsibilities

• Visit patients in their homes and provide skilled nursing care according to the established plan of care.
• Implement physician-ordered and clinician-directed care plans.
• Monitor and evaluate the effectiveness of patient care and report changes in condition.
• Accurately document patient assessments, treatments, and medical history.
• Provide emotional support, education, and encouragement to patients and their families.
• Complete and submit timely progress notes and visit documentation daily.
• Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure quality patient outcomes.
• Promote patient safety, comfort, and independence.

## Qualifications
**Knowledge and Skills:**

• Knowledge of infection control policies and procedures, i.e., Standard Precautions, Isolation Precautions, personal protective equipment, hand hygiene.
• Ability to work in an interdisciplinary team, supervised by a nurse
• Knowledge of the delivery of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.

**Qualifications:**

• Current Washington State LPN or Multistate LPN license
• 6 months of LPN experience
• Ability to drive with clean driving record
• Home health/hospice experience preferred
