### What You'll Do

• Coordinate and manage hospice care for patients throughout Gulf County.
• Complete admissions, assessments, and ongoing case management.
• Collaborate with physicians, hospice aides, social workers, chaplains, and other clinical professionals.
• Help patients and families navigate one of life's most important journeys with dignity, compassion, and expert clinical care.

## We're Looking For Candidates with

✔ Florida Registered Nurse (RN)

✔ Hospice, home health, oncology, long-term care, or strong medical-surgical experience preferred

✔ Compassionate communicator and patient advocate

✔ Self-motivated professional who enjoys working independently while being part of a collaborative team
