**Position Title: Community Support Professional (CSP)**

Reports To: Director / CEO

Employment Type: Full-Time, Salaried

Annual Salary: $45,760

Minimum Billing Requirement: 15 billable hours per week

**Position Summary**

The Community Support Professional (CSP) provides person-centered support services to individuals with disabilities, focusing on skill-building, community integration, functional independence, and overall quality of life.

***Essential Duties and Responsibilities***

**Individual Support & Skill Development**

  • Provide direct support based on the Individualized Support Plan (ISP) or Person-Centered Plan (PCP).
  • Deliver skill-building activities to promote independence across life skills.
  • Assist individuals in accessing community activities, appointments, and meaningful opportunities.
  • Document progress in approved agency systems.

**Billable Service Requirements**

  • Maintain a minimum of 15 billable hours per week.
  • Submit documentation timely and accurately for billing compliance.
  • Notify Director/CEO immediately if unable to meet required billable hours.

**Compliance & Documentation**

  • Complete all service notes and entries within required timeframes.
  • Maintain confidentiality and follow HIPAA and DDS standards.
  • Ensure all services meet DDS, Medicaid waiver, and agency compliance criteria.

**Professionalism & Communication**

  • Maintain professional communication with individuals, families, and the care team.
  • Attend trainings, team meetings, and required supervision.
  • Report incidents or status changes to leadership immediately.

**Safety & Risk Management**

  • Follow all agency safety guidelines and risk procedures.
  • Support individuals safely during community and home activities.
  • Respond to emergencies as required.

## Qualifications
**Qualifications**

  • High school diploma required; Associate’s or Bachelor’s preferred.
  • Experience supporting individuals with disabilities is strongly preferred.
  • Valid driver’s license, insurance, and reliable transportation.
  • Ability to complete all required agency and DDS trainings.
  • Strong communication, documentation, and time-management skills.

**Work Schedule**

  • Full-time salaried position with flexibility based on client needs.
  • Evening/weekend hours may be required.
  • Billable hour minimums and documentation deadlines must be met.

**Physical Requirements**

  • Ability to lift 25–50 lbs and assist with mobility.
  • Ability to drive, walk, stand, and support individuals in varied environments.
