At Countryside Senior Living and Memory Care of Wichita Falls, the Resident Care Partner provides compassionate, person-centered care to residents living with dementia. This role focuses on building meaningful relationships, promoting dignity and independence, and enhancing each resident's quality of life through individualized care, companionship, and purposeful engagement. The Resident Care Partner works closely with families and the interdisciplinary team to understand each resident's life story, preferences, and behavior to provide personalized support and successful behavioral interventions.

**Key Areas of Responsibility:**

**Person-Centered Care**

• Provide individualized, person-centered care that respects each resident's dignity, preferences, and abilities.
• Build trusting relationships with residents through meaningful companionship and consistent engagement.
• Deliver one-on-one care and emotional support to promote comfort, security, and well-being.
• Encourage independence while providing assistance based on each resident's needs.

**Resident Engagement**

• Plan, record and participate in daily activities that promote:
• Cognitive stimulation
• Social interaction
• Physical movement and exercise
• Emotional well-being
• Encourage resident participation in activities that support purpose, identity and quality of life.
• Adapt activities to each resident's cognitive and physical abilities.

**Behavioral Support**

• Observe and recognize changes in behaviors, mood, and functioning.
• Utilize dementia care best practices to redirect, de-escalate, and manage behaviors in a calm and supportive manner.
• Document observations and communicate changes to the nursing and leadership team.
• Maintain a safe, reassuring, and structured environment.

**Family Communication**

• Build positive relationships with residents' families.
• Gather information regarding each resident's life history, routines, preferences, interests, and behavioral triggers.
• Share appropriate updates with families and collaborate with the care team to enhance individualized care plans.

**Personal Care Assistance**

Aid with Activities of Daily Living (ADLs), including but not limited to

• Bathing
• Dressing
• Grooming
• Toileting
• Continence care
• Eating and hydration assistance
• Ambulation and transfers
• Positioning and mobility assistance

**Team Responsibilities**

• Work collaboratively with nursing staff, therapists, activity personnel, and leadership.
• Follow all community policies, infection control standards, and safety procedures.
• Maintain accurate documentation as required.
• Attend required training and continuing education, including dementia-specific education.
• May require weekends, Holidays, and overtime as needed.

## Qualifications
**Education**

• High School Diploma or GED preferred.

**Experience**

• Minimum of 1 year of caregiving experience, preferably in memory care, assisted living, long-term care, or home health.
• Experience caring for individuals living with Alzheimer's disease or other forms of dementia is strongly preferred.

**Knowledge, Skills, and Abilities**

• Understanding of person-centered dementia care principles.
• Excellent communication and interpersonal skills.
• Ability to build trusting relationships with residents and families.
• Strong observation and behavior management skills.
• Ability to remain calm and compassionate during challenging situations.

**Physical Requirements**

This position is physically demanding and requires the ability to

• Frequently stand and walk for extended periods.
• Bend, stoop, kneel, squat, reach, and twist.
• Assist residents with transfers and mobility.
• Lift, push, pull, and carry 25-50 pounds safely.
• Respond quickly during resident emergencies.
• Perform repetitive movements throughout the workday.
