About The Role
## Description
The Aging Services Program Manager will
- Be responsible for providing coordination of services and outreach to older adults who are registered
to Isaacs Center programs.
- Provide support in maintaining behavioral and operational challenges that arise.
- Develop, maintain, and continually update a schedule of programs and supportive services in
alignment with the vision and contractual obligations for our NYC Aging Contracts
- Serve as liaison between specialists, consultants, volunteers, interns, supplemental service providers,
property management, and other community stakeholders.
- Conduct outreach to share program services and offerings to new and potential members.
- Supervise program support staff.
- Ensure compliance with NYC Aging, NYSOFA, and Internal requirements as needed.
- Work closely with NORC Program Manager to create programming opportunities and increase
Salary: $66,300- $70,000

- Ensure participants gain access to social services, wellness education, arts and culture, and recreation
- activities
- Create and manage a monthly calendar of activities, services, and events
- Develop and maintain relationships with community partners and consultants
- Develop professional provider relationships with older adults, families and caretakers
- Develop individualized service plans and reassess regularly
- Develop and maintain a directory of supportive services and agencies available locally
- Organize and lead community center trips and outings · Assist with outreach efforts and follow-up to referrals
- Advocate for clients' rights, entitlements, and benefits · Communicate with hospital and rehabilitation social workers when needed
- Submit weekly Dashboard and Service Log reports and maintain documentation Work with supervisor and government agencies to ensure compliance
- Provide support for programs through planning, implementation, management, and evaluation
Qualifications
- The Aging Services Program Manager must possess strong writing, speaking, interpersonal, and
- organizational skills. It is preferred that the candidate hold a bachelor's degree. The candidate must
- have demonstrated experience providing group facilitation, maintaining strong interpersonal skills, and
- managing data and outcomes. The candidate must have a history of working with diverse and high-needs
- aging populations, value team building, and support the development of a community to address complex
- issues.
- Bachelor's degree
- Prior experience working with the elderly highly preferred
- Bilingual, English/Chinese or English/Spanish highly preferred
- Ability to handle confidential information with discretion.
- Ability to work some flexible hours
- Working knowledge of community resources and service providers to older adults
- Ability to work effectively with cultural, economically and educationally diverse populations
- Strong verbal, written and interpersonal communication skills.
- Demonstrated knowledge of aging-related issues.
- Computer literacy includes Microsoft Word, Excel, and Outlook
