**Habilitation Aide****–** *ICF Fairfax*

*Clearbrook has three **Intermediate Care Facilities** (aka **ICF’s**), each housing multiple clients. Goals are developed for each client based on their needs. Programming is assigned, usually pertaining to areas such as self-care, along with community living & integration. Each facility has a nurse on staff at least 20 hours per week to coordinate any medical needs required by the clients.*

**Location**: Rolling Meadows

**Hours**: Part Time

**Schedule**: Wednesday 6am-10am & 2:30pm-11pm, Sunday 9am-9pm (24hrs)

**Pay**: $21.35/hour + Paid Training!

**Med Passer Differential**: + $1/hr during medication pass periods

**Weekend Pay:** + $3/hr differential

**Responsibilities**

• Responsible for the direct provision of active treatment to the clients including training and assistance in all areas addressed in the individual program plans of the clients to whom she/he is assigned.
• Teach and provide the support necessary for the individual’s use of community resources.
• Assist clients with daily living activities including (but not limited to) personal hygiene, feeding & housekeeping.
• Assist clients in all areas of social and recreational program activities for the day.

## Qualifications

**Requirements**

• High School diploma or equivalent
• Valid IL driver’s license required
• Must be **21** years of age
