**Supported Living Assistant - Crystal Lake**

The Supported Living Assistant provides support and service to clients who have intellectual disabilities living in their natural home environment. This part-time position consists of working 1:1 with an individual in their own home and community. Qualified persons will have own reliable transportation, valid driver’s license, vehicle insurance, and a high school diploma.

**Hours**: Up to 15 hours per week

Monday: 2:30pm-4:00pm

Tuesday: 2:30pm-6:30pm

Wednesday: 2:30pm-6:30pm

Thursday: 2:30pm-4:30pm

Friday: 2:30pm-6:30pm

**Pay**: $21.35/hr

**Responsibilities**

  • Provides support and service to clients living in the community in their natural home environment

## Qualifications
**Requirements:**

  • High school diploma or equivalent
  • Valid driver’s license required
  • Must be at least 18 years of age
