**Job Summary**
We are seeking an empathetic, highly organized, and resilient Patient Care Coordinator to join our administrative hospice team. In this role, you will serve as the central hub of communication, ensuring our patients and their families receive the dignity and care the deserve. You will manage daily staff scheduling, track critical compliance metrics, handle incoming inquiries from families in crisis, and maintain precise, up-to-date patient records.
**Core Responsibilities**
- **Care Scheduling:** Coordinate schedules for call-outs & PRN coverage to ensure visits align with current frequencies
- **Visit tracking:** Track HOPE visits to identify gaps and reinforce required timelines
- **Interdisciplinary Group (IDG) Prep:** Compile and generate the IDG report
- **Chart Accuracy:** Update patient charts to help ensure accuracy for field staff
- **Visits Auditing:** Monitor ancillary staff visits to guarantee plan-of-care compliance
- **Utilization Management:** Process and track Level of Care changes
- **Intake & Triage:** Answer urgent calls from families, assess needs calmly, and route information immediately to clinical staff

## Qualifications
- **Experience:** 1–2 years of healthcare administrative experience (hospice or home health preferred)
- **Education:** High school diploma required
- **Tech Skills:** Proficiency with EMR software (FireNote) and Microsoft Office
- **Core Competencies:** Requires advanced problem-solving skills and the ability to use effective interpersonal relations and communication skills
- **Mission Alignment:** Actively promotes and advocates for the hospice philosophy of care
- **Physical Demands:** Prolonged sitting and some standing are required. Occasional need to lift, pull, carry, and push items weighing up to fifty (50) pounds (such as medical supply boxes)
- **Work Environment:** Requires working under some stressful conditions to meet tight regulatory and clinical deadlines
