About The Role
The Transitional Care nurse provides call post discharge to ensure understanding of discharge care and assist with follow up PCP appointment. Will assist patient/caregiver with obtaining PCP and the appropriate SDOH needed if applicable. The patient will be followed weekly up to 4weeks based on needs and support
