## ❤️ What You'll Do

As our **Residential Living Social Worker**, you'll play a vital role in helping residents navigate life's transitions while promoting independence, wellness, and quality of life.

Your responsibilities will include

• 🏡 Coordinating admissions and welcoming new Residential Living residents
• 🤝 Serving as an advocate and trusted resource for residents and their families
• 💙 Assisting residents with transitions, discharges, and moves throughout the community
• 🩺 Collaborating with our Wellness Team, Personal Care, Skilled Nursing, Marketing, and Life Enrichment departments
• 📋 Helping resolve resident concerns while promoting independence and resident satisfaction
• 🎉 Planning wellness programs, educational opportunities, support groups, and resident events
• 📞 Communicating with outside providers and coordinating services to meet resident needs
• 📝 Monitoring resident well-being, documenting incidents, and ensuring compliance with community policies
• ✨ Supporting a warm, engaging, and person-centered environment where residents feel valued every day
