About The Role

## Description

The Nursing Supervisor – Day & Community Services serves as the clinical lead for HeartShare's Day Habilitation and Community Services programs, providing oversight of nursing operations, regulatory compliance, medication management, infection prevention, and healthcare coordination. This position supervises nursing staff across multiple locations and partners closely with program leadership, healthcare providers, and interdisciplinary teams to ensure the delivery of high-quality, person-centered services.

The role is responsible for promoting the health, safety, and well-being of individuals with intellectual and developmental disabilities while ensuring compliance with OPWDD, Department of Health, Medicaid, Justice Center, and agency requirements. As a key member of the leadership team, the Nursing Supervisor supports quality improvement initiatives, maintains clinical standards, and helps drive positive health outcomes across all Day & Community Services programs.

**Nursing Leadership & Program Oversight**

  • Provide nursing leadership and clinical oversight across all Day & Community Services programs.
  • Supervise, mentor, coach, and support nursing staff through regular supervision meetings, field observations, and performance evaluations.
  • Assist with recruitment, onboarding, orientation, training, and retention of nursing personnel.
  • Ensure nursing coverage and staffing plans adequately support program operations.
  • Develop, implement, and maintain standardized nursing practices, clinical procedures, and healthcare protocols.
  • Promote a culture of professionalism, accountability, collaboration, inclusion, and continuous improvement.
  • Serve as the primary nursing resource for Program Directors, Assistant Directors, Supervisors, and Direct Support Professionals.
  • Collaborate with operational leadership to support participant health, safety, and person-centered service delivery.
  • Participate in strategic planning, quality improvement initiatives, and program development efforts.

**Clinical Oversight & Healthcare Coordination**

  • Ensure implementation of physician orders, treatment recommendations, and nursing interventions.
  • Review health assessments, nursing evaluations, medication records, and healthcare documentation for accuracy and compliance.
  • Monitor participant health needs and coordinate healthcare services, appointments, and follow-up care.
  • Support individuals with chronic medical conditions and complex healthcare needs.
  • Monitor participant health trends and identify opportunities for preventative healthcare interventions.
  • Coordinate communication among physicians, specialists, hospitals, pharmacies, care managers, families, residential providers, and program teams.
  • Participate in interdisciplinary meetings, case conferences, and person-centered planning meetings.
  • Ensure healthcare supports and safeguards are effectively implemented and monitored.
  • Assist programs in addressing participant health concerns, behavioral health needs, and medical emergencies.

**Public Health, Infection Prevention & Department of Health Compliance**

  • Serve as the clinical lead for infection prevention and public health initiatives throughout Day & Community Services.
  • Ensure compliance with New York State Department of Health (DOH) regulations, guidance, and reporting requirements.
  • Monitor communicable disease trends and coordinate appropriate response measures.
  • Ensure timely reporting of communicable diseases, outbreaks, and reportable health events as required by DOH regulations.
  • Develop and oversee infection prevention and control protocols.
  • Coordinate emergency preparedness efforts related to public health concerns and infectious disease management.
  • Educate staff regarding infection prevention, disease transmission, vaccination recommendations, personal protective equipment (PPE), and public health best practices.
  • Collaborate with local and state public health agencies when required.
  • Monitor compliance with health screenings, immunization documentation, and infection control standards.
  • Assist in implementing public health directives and emergency response measures.

**AMAP Administration & Medication Management**

  • Oversee all Approved Medication Assistive Personnel (AMAP) activities within Day & Community Services.
  • Ensure compliance with OPWDD medication administration regulations and agency policies.
  • Monitor AMAP certification, recertification, competency assessments, and required documentation.
  • Coordinate medication administration observations and quality assurance reviews.
  • Conduct medication audits and review medication error reports.
  • Develop corrective action plans related to medication administration concerns.
  • Collaborate with AMAP instructors and training providers to maintain staff competency.
  • Monitor medication storage, documentation practices, and medication room compliance.
  • Identify and mitigate medication administration risks through proactive oversight and staff education.

**Regulatory Compliance & Quality Assurance**

  • Ensure compliance with all applicable federal, state, local, and agency regulations, including:

  • Conduct nursing audits, medication audits, healthcare record reviews, and compliance monitoring activities.
  • Review documentation to ensure accuracy, completeness, timeliness, and regulatory compliance.
  • Participate in internal audits, external reviews, OPWDD surveys, DOH inspections, and regulatory visits.
  • Develop, implement, and monitor corrective action plans.
  • Ensure programs remain in a constant state of audit and survey readiness.
  • Analyze healthcare-related trends and recommend quality improvement initiatives.
  • Maintain required records and reports related to nursing services and healthcare compliance.

**Health, Safety & Risk Management**

  • Promote a safe and healthy environment for all individuals receiving services.
  • Review health-related incidents, accidents, injuries, illnesses, medication errors, and healthcare concerns.
  • Ensure timely incident reporting and follow-up in accordance with agency and regulatory requirements.
  • Assist with investigations involving health and safety concerns.
  • Identify healthcare-related risks and implement preventative interventions.
  • Support emergency preparedness planning and response activities.
  • Ensure emergency medical supplies, first aid equipment, and healthcare resources are properly maintained.
  • Collaborate with program leadership to reduce risk and improve participant health outcomes.
