About The Role

## Description
**Status:** Full-time, Hourly - Rotating weekends, Early morning shifts 6a-2p
**Responsibilities:**
# *1. Residential Health Care*

  • Conduct in-home visits to make periodic direct observation of residents; including watching during activities of daily living, tracking progress, and updating health records. Contact RN as needed for direction and/or consultation.
  • Collect objective data for resident assessments as delegated by the RN.
  • Maintain and update resident health records, including: medical/dental forms
  • annual dental, physical, and vision forms
  • monthly weights
  • medications
  • medication verification forms/standard orders
  • Report suspected abuse/neglect of residents, with systemic recommendations to prevent future occurrences.
  • Facilitate communication with outside health facilities such as pharmacies, clinics, hospitals, etc.

## 2. Direct Care

  • Provide direct care services as outlined in the Direct Support Professional (DSP) job description.
  • Provide professional guidance and feedback to direct care staff as it pertains to the health and wellness of the individuals receiving services.

## 3. Staff Education and Training

  • Monitor medication administration techniques of unlicensed personnel using an objective format developed by the RN. Data will be reported to the RN for review and evaluation.
  • As a certified instructor, periodically teach First Aid/CPR to Range Center, Inc. staff members as a part of the Range Center, Inc. staff training program.

## Qualifications
**Qualifications:**

  1. Hold a current license by the Minnesota board of nursing to practice practical nursing.
  2. Hold a current instructor certification in First Aid/CPR.
  3. Demonstrate organizational skills, excellent oral and written communications, and ability to work with a variety of people.
  4. Preferably have at least one year of experience in the field of developmental disabilities.
  5. At least 8 hours of applicable in-service training annually; including Range Center, Inc. vulnerable adult abuse prevention policy and procedures
