Greenfield Rehabilitation Agency is looking for a **passionate PRN Physical Therapist** to join our team supporting **senior communities in Pigeon Falls and surrounding areas**. As a PRN PT, you’ll provide **skilled, patient-centered care** across **skilled nursing facilities, assisted living communities, and in-home care**, helping residents improve mobility, strength, and independence while enhancing quality of life.

### **Your Role:**

• Conduct initial and ongoing **evaluations** to assess mobility, balance, strength, and function
• Develop and implement **individualized treatment plans**
• Provide hands-on therapy to help residents **recover, maintain independence, and prevent falls**
• Educate residents, families, and staff on **safe mobility and functional activities**
• Collaborate with **PTs, OT, SLPs, nurses, and other staff** to ensure coordinated, holistic care
• Maintain accurate **documentation** of therapy sessions and outcomes

### **Requirements:**

• Licensed or eligible for licensure as a **Physical Therapist in Wisconsin**
• Strong clinical skills with a passion for helping older adults
• Excellent communication, collaboration, and problem-solving abilities

## Qualifications

• Graduate of an accredited school with a bachelor’s, master’s, or doctoral degree in Physical Therapy.
• License as required by the State of Wisconsin to practice as a Physical Therapist.
