Greenfield Rehabilitation Agency is excited to announce an opportunity for a **Certified Occupational Therapy Assistant (COTA)** to join our therapy team in Elkhorn and surrounding areas.

### **Your Responsibilities**
- Deliver compassionate and effective occupational therapy treatments under OT supervision
- Implement individualized treatment plans to improve functional independence and quality of life
- Support residents in achieving personal goals and progress milestones
- Accurately document care and maintain compliance standards (experience with Net Health a plus)
- Collaborate with the therapy and nursing teams to ensure patient-centered outcomes
- Educate residents and families on safety and daily living strategies
- Participate in team meetings and contribute to care planning

## Qualifications
- Graduate of an accredited school with an associate degree as an Occupational Therapy Assistant.
- Certification by the State of Wisconsin to practice as an Occupational Therapy Assistant.
