## Description
**Job Summary:**

• Facilitate interactions between patients and their physicians.
• Provide logistical support for patients needing diagnostic and/or treatment planning.
• Secure referrals and assist with financial and insurance issues.
• Provide guidance and coordination of services for patients and families across the continuum of care.
• Assists the Director of Case Management with daily operations and other responsibilities as needed.
• Demonstrate and encourage team behavior and exceptional patient/guest experiences.
• Uphold and promote patient safety and quality.

## Qualifications
**Education:**
High school diploma or GED required; Associate's degree in healthcare administration, business, social services, or a related field preferred.
**Experience:**
Minimum of two years of experience in a healthcare, medical office, customer service, patient access, care coordination, or similar patient-facing role preferred.
**Additional Skills/Abilities:**
Demonstrated knowledge of healthcare terminology, hospital services, insurance processes, and referral procedures preferred.
