## Description
**Job Summary:**

• Provide direct personal hygiene and care to the home care/hospice patient according to written plan of care.
• Document all patient encounters and care according to policy.
• Demonstrate and encourage team behavior and exceptional patient/guest experiences.
• Uphold and promote patient safety and quality.

## Qualifications
**Education:**
High school graduate of GED. Must have certification as a certified nursing aide.
**Experience:**
One year of healthcare experience preferred.
**Additional Skills/Abilities:**
Good communication and organizational skills.
