About The Role
## Description
The Case Manager coordinates services for individuals with brain injuries to support their highest level of independence in community living. This role focuses on person-centered planning, advocacy, and service coordination to ensure each individual’s safety, health, and personal goals are achieved.
Primary Responsibilities
- Conduct assessments and develop person-centered service plans
- Support individuals’ choices, preferences, and self-determination
- Identify and connect clients to appropriate community resources
- Advocate for necessary medical, social, and educational services
- Coordinate and access waiver and non-waiver funding sources
- Empower individuals to build personal responsibility and independence
- Monitor and evaluate plans of care to ensure safety and well-being

## Qualifications
**Experience:**
Graduate from an accredited four-year college with a major in social work, psychology, sociology, or a closely related field; or a graduate from an accredited four-year college with a major in any field and one year experience as a social worker in a public or private social service agency. Experience working in Adult Foster Care, Independent Living Skills (ILS), ARMHS, or supported employment is a plus.
