Discharge Planner

## Description

The Discharge Planner is responsible for coordinating and facilitating safe, timely, and effective resident discharges from the long-term care facility. This position collaborates with residents, families, physicians, nursing staff, therapy, social services, and community providers to ensure continuity of care and successful transitions to home, assisted living, hospitals, or other healthcare settings. The Discharge Planner ensures compliance with federal and state regulations while promoting positive resident outcomes

## Qualifications

• Knowledge of Medicare, Medicaid, managed care, CMS regulations, and long-term care reimbursement.
• Strong knowledge of community resources and post-acute care services.
• Excellent communication, organizational, and problem-solving skills.
• Proficient in electronic medical records and Microsoft Office applications.
