**Occupational Therapist (OT) Home Health – Full-Time**

**Location:** Keller, Bedford, North Richland Hills, Hurst, Roanoke, Flower Mound, TX

**TheraCare** is seeking a licensed Occupational Therapist (OT) for a Full-Time home health opportunity providing in-home physical therapy services to adult and geriatric patients throughout Keller, TX and surrounding areas.

This Full-Time home health OT position is ideal for clinicians looking for flexibility, autonomy, and the ability to provide one-on-one patient care in the home setting. **Job Responsibilities**

• Perform home health occupational therapy evaluations and re-evaluations for adult and geriatric patients
• Develop and update individualized Plans of Care (POC)
• Treat patients with orthopedic, neurological, cardiac, post-surgical, or other medical conditions
• Address ADLs, IADLs, coordination, strength, balance, and functional mobility
• Apply manual therapy techniques and non-complex modalities, including heat, cold, electrical stimulation, and other physician-ordered treatments
• Implement interventions for fall prevention, home safety, and environmental modifications
• Train patients and caregivers in adaptive equipment and durable medical equipment (DME)
• Fabricate splints and recommend assistive devices as appropriate
• Complete accurate and timely documentation in the EMR
• Communicate with physicians and interdisciplinary care teams
• Supervise COTAs and home health aides, as applicable
• Follow all home health regulations, infection control, and safety standards

## Qualifications
**Qualifications**

• Doctorate in Occupational Therapy from an accredited program
• Active Occupational Therapist (OT) license in the state of Texas or eligible Compact OT Licensure
• Home Health experience (1 year required)
• HomeCareHomeBase (HCHB) experience (preferred)
• Current CPR certification
• Strong communication, clinical judgment, and time-management skills
• Knowledge of home health regulations and Medicare guidelines preferred
• Valid driver’s license, reliable transportation, and current auto insurance
