**POSISTION SUMMARY**

Provides direct case management services to clients. Assists in the interviewing, training, and ongoing support to social workers and social services specialists in the Permanent Supportive Housing Program. Intervenes and assists case managers in working through complex client situations and client crisis events. Collaborates with PSH Supervisor and Director of Client Services to develop short-term and long-term plans to implement and facilitate the achievement of department goals and quality improvement projects. Assists the Director of Client Services in meeting all regulatory and quality standards. Maintains a close working relationship with direct supervisor, participates in active supervision, and remains receptive to feedback on professional development to support high quality service provision.

**KEY DUTIES AND RESPONSIBILITIES**

**RESPONSIBILITIES**

  • Provides housing case management support to clients on assigned caseload so that they might maintain their housing.
  • Supports PSH supervisor in daily program tasks and provides back-up when unavailable
  • Assists PSH supervisor to hire and train new staff and provide ongoing training to existing staff
  • Assists housing case managers to work through complex client situations and manage crises.
  • Collaborates with PSH supervisor, Director of Client Services and other staff to develop and implement program goals and regularly revise them based on ongoing quality improvement activities.

**ACCOUNTABILITIES**

  • Provides direct services to clients and maintains a caseload.
  • Attends all meetings and trainings as directed by supervisor.
  • Maintains training, knowledge, and skills through remaining current on ASIST and MHFA certifications in order to intervene and assist with clients reporting feelings of SI, HI, or other crisis situations.
  • Assists with identifying and coordinating case management training by community partners or other local agencies that case management may utilize through referral or knowledge sharing in order to meet client needs.
  • Coordinates with supervisor to ensure case managers receive correct and consistent training and updates. Assists PSH Supervisor with initial training of new staff by providing shadowing opportunities and check-ins. Assists with documentation of training activities and reports to PSH Supervisor to assist in resolution of personnel issues.
  • Provides ongoing support to case managers by offering guidance on best practices and approaches related to client services.
  • Coordinates with supervisor to ensure program is appropriately staffed, participates in interviews, and consults on case management staffing decisions.
  • Assists PSH Supervisor in ensuring staff are clear on program expectations and standards, including but not limited to intake and service provision, record keeping, and documentation, behavioral expectations, work quality and productivity expectations, effective teamwork, and client-centered ethical practice.
  • Works with supervisor to ensure that funder and accreditor requirements are being met (LDH, Medicaid, HOPWA, UNITY, etc) by assisting with auditing client charts for completeness. Maintains documentation of charts reviewed and reports any discrepancies to Social Services Supervisor.
  • Participates in community and agency/program meetings, team meetings, and staff in-services.
  • Other duties as assigned by supervisors.

**Qualifications**
# **POSISTION REQUIREMENTS**

  • The minimum education level required for the position is a Master’s in Social Work (MSW).
  • The minimum experiential level required for the position is five years in social work services and a minimum of three years of housing-related experience.
  • The required license is an Registered Social Worker (RSW) or Certified Social Worker (CSW) in good standing.
  • Minimum age of 21.
  • Must have reliable transportation and a valid driver’s license, no license denials, revocations, or more than one suspension in the past three years; valid car insurance; no more than one at-fault accident in the past three years and no more than two moving violations in the past five years.

**ESSENTIAL PHYSICAL REQUIREMENTS**

  • Extended periods of sitting required.
  • Work is primarily office based.
  • Some travel may be required.
  • May need to lift up to 25 pounds in equipment and supplies.
