**Summary:**

The Outreach and Navigation Manager provides strategic leadership for NOHN's outreach, community engagement, patient navigation, and social needs resource programs. As a key leader within a Federally Qualified Health Center (FQHC), this position develops and implements initiatives that improve access to care, reduce barriers to health services, strengthen community partnerships, and support fair access to healthcare resources for underserved and vulnerable populations.

The Outreach and Navigation Manager oversees community outreach efforts, Community Health Workers, Resource Navigators, and related programs designed to address social drivers of health and improve patient engagement. This role collaborates across departments and with community partners to ensure outreach and navigation services align with NOHN's mission, strategic priorities, quality initiatives, and population health goals.

The Manager serves as an ambassador for NOHN throughout the community, fostering meaningful partnerships that help improve health outcomes and strengthen access to comprehensive healthcare services throughout Clallam and Jefferson Counties.

**Strategic Leadership**

  • Develop, implement, and evaluate outreach, patient navigation, and community engagement strategies that support NOHN's mission, vision, strategic plan, and population health goals.
  • Provide leadership and oversight for outreach staff, Community Health Workers, Resource Navigators, and other assigned personnel.
  • Establish annual goals, performance metrics, and quality improvement initiatives related to outreach and navigation activities.
  • Monitor program effectiveness and utilize data to identify opportunities for improvement and growth.
  • Participate in strategic planning initiatives related to access, population health, social drivers of health, and community engagement.

**Community Outreach and Partnership Development**

  • Develop and maintain collaborative relationships with healthcare organizations, social service agencies, schools, tribal partners, community-based organizations, businesses, local governments, and other stakeholders.
  • Identify partnership opportunities that expand access to healthcare and supportive community services.
  • Represent NOHN at community meetings, coalitions, health fairs, outreach events, and regional initiatives.
  • Coordinate and oversee clinic-sponsored community events, including health fairs, prevention programs, and outreach activities.
  • Serve as a primary liaison between NOHN and community organizations seeking collaboration opportunities.

**Navigation and Social Drivers of Health Programs**

  • Lead organizational efforts related to identifying and addressing social drivers of health that impact patient outcomes and healthcare access.
  • Oversee the development, implementation, and continuous improvement of navigation workflows, screening tools, referral processes, and resource coordination programs.
  • Ensure patients are connected to community resources that support housing stability, food security, transportation, utilities assistance, insurance enrollment, employment services, behavioral health support, and other essential needs.
  • Maintain oversight of community resource databases and referral resource networks.
  • Collaborate with clinical, behavioral health, care management, and quality teams to integrate navigation services into patient care workflows.

**Access and Patient Engagement**

  • Develop initiatives that improve patient access to primary care, behavioral health, dental services, specialty care, and preventive health services.
  • Collaborate with health plans, community partners, and internal stakeholders to strengthen patient engagement strategies.
  • Lead outreach efforts targeting preventive care, chronic disease management, health education, insurance enrollment, and patient retention.
  • Support organizational efforts to reduce barriers to care and promote fairness in healthcare access and service delivery.

**Mobile Health and Community-Based Services**

  • Serve as the primary point of contact for community organizations interested in partnering with NOHN's Mobile Health Clinic.
  • Assess community needs and coordinate mobile health deployment opportunities in collaboration with operational leadership.
  • Expand outreach efforts to underserved, rural, and hard-to-reach populations through innovative service delivery models.

**Quality Improvement and Compliance**

  • Collaborate with Quality, Compliance, and Leadership teams to support Community Health Needs Assessments and related initiatives.
  • Ensure outreach and navigation programs comply with HRSA requirements, FQHC regulations, grant requirements, and organizational policies.
  • Develop and monitor performance indicators related to outreach, navigation, patient engagement, and community partnerships.
  • Prepare reports, presentations, and recommendations for organizational leadership.

**Staff Leadership and Development**

  • Recruit, train, mentor, coach, and evaluate assigned staff.
  • Foster a culture of accountability, collaboration, innovation, and service excellence.
  • Support professional development opportunities for outreach and navigation staff.
  • Conduct regular team meetings and ensure effective communication across departments.

**Collaboration and Relationship Management**

  • Build strong relationships with providers, clinic leadership, care teams, and community partners.
  • Participate in multidisciplinary care management meetings and community collaborative forums.
