Overall Function

Aids program clients daily with the implementing of Individual Support Plans in the client’s home and community.

Essential Functions

• Provides hands-on instruction to in-home client to implement Individual Support Plans and client development in the home and community.
• Conducts recreational and social activities with individuals, as assigned.
• Participates in regular staff training to develop basic knowledge and skills necessary to assist individuals.
• Responds appropriately to emergency situations and ensures the health and safety of the client.
• Other duties as required.
• Adheres to all policies, laws, regulations and codes of ethics and confidentiality as outlined by federal and state laws, and agency policies and procedures.

## Qualifications

• Ability to receive, understand, and communicate information with effectiveness, and follow instructions.
• Must successfully obtain Criminal History Clearance from the Pennsylvania State Police and Child Abuse Clearance from the Pennsylvania Department of Human Services and FBI Fingerprint Clearances.
• Must not be excluded from participation in Medicare, Medicaid or any other federal health care program.
• Compliance with pre-employment and routine drug testing is required.
• Ability to move, assist and/or transfer individuals weighing up to 150 pounds may be required.
