**About the Health Care Home Program**

Our team promotes preventive care, healthy living, and health education for clients and staff alike. As a Nurse Care Manager, you’ll be at the heart of client wellness, supporting individuals with chronic conditions and behavioral health needs.

**What You’ll Do**

  • Collaborate with an interdisciplinary team to develop client-centered wellness plans
  • Conduct health and metabolic screenings (quarterly or annually)
  • Provide outreach calls to engage and support clients
  • Lead health and wellness education for clients
  • Monitor hospital admissions/discharges and coordinate medication reconciliation
  • Partner with healthcare providers to ensure continuity of care
  • Track and assess performance measures related to physical health
  • Occasionally (up to 25%) meet clients in the community or in their homes

**Requirements of the Position**

  • Graduation from an accredited nursing program
  • Active, unrestricted Missouri LPN or RN license
  • Valid driver’s license, proof of insurance, and a reliable vehicle
  • (Preferred) 2+ years of nursing experience in a mental health setting
  • Familiarity with mental illness and substance use disorders
  • Strong communication and customer service skills
  • Comfort with electronic health records and intermediate computer proficiency (Microsoft Office and use of email)
