The **Assertive Community Treatment (ACT) Team** provides comprehensive, full-support mental health services for individuals living with severe and persistent mental illness, such as Schizophrenia or bipolar disorder. ACT goes beyond traditional outpatient care by offering intensive treatment, rehabilitation, and support services in the community.

• Medication management and education
• Client assessment and evaluation

• Documentation that meets regulatory standards
• Supporting clients in maintaining treatment and medication regimens

• **Full-Time Hours:** Monday–Friday, 8:30 AM – 4:30 PM
• **On-Call:** May be required after hours for client care

## Qualifications
#### **Qualifications**

• Graduate from an accredited School of Nursing (Diploma, Associate, or Bachelor)
• Valid **PA Registered Nurse License**
• **Preferred:** 1+ year of mental health nursing experience
• **Other Requirements:**
• PA State Criminal, Child Abuse, and FBI Clearances
• Valid Driver’s License and acceptable Motor Vehicle Record
