About The Role
## Description
**Job Purpose:**
---
Provides case management by providing team-oriented case management services for Rawhide clients; coordinating the overall interdisciplinary plan of care for clients, from admission to discharge and aftercare; acting as a liaison between client/family, placing agency, and Rawhide treatment team, to ensure necessary care is provided promptly and effectively; overseeing alumni services, needs, requests, and scholarships.
**Job Responsibilities:**

- Ensures the therapeutic needs of clients and families are met by working collaboratively with the therapist and living unit personnel; advising individual clients; representing issues, needs, and progress; ensuring the family attends appropriate therapeutic activities.
- Provides crisis support by maintaining availability to residents and living unit staff when called upon; collaborating with the living unit, support staff, and management team regarding the best course of action to help stabilize; developing methods to restore residents to the therapeutic environment.
- Acts as a liaison to placing agencies and families by establishing and maintaining mutual understanding, cooperation, and collaboration; coordinating progress review meetings with parents and social workers; working collaboratively with internal care providers to ensure the needs of the clients are being met; keeping team members informed about goal progress, obstacles, treatment plan changes, and schedules.
- Provides case management to support residents by developing treatment plans and aftercare; evaluating progress through written reports such as progress reports and discharge summaries; measuring results against standards and making necessary adjustments; establishing and maintaining cooperation and information flow between internal and external resources; assisting in identifying needed independent living skills; leading treatment team meetings and intervention meetings.
- Maintains Rawhide's stability, reputation and mission by ensuring that case management of client is in keeping with the goals and philosophies of Rawhide, and state licensing regulations; participating in weekly Case Management meetings.
- Provides aftercare services by advising clients and alumni; assessing client needs and ensuring client has full resources and services available for their transition into the community; assisting the client in transitioning back into the community through established aftercare plan; communicating needs to Case Management Supervisor; completing and routing alumni request-for-assistance forms and scholarship applications; documenting contacts, requests, and scholarships in Harmony.
- Maintains historical reference by following procedures for retention, protection, retrieval, transfer, and disposal of records.
- Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; keeping informed of state licensing rules; establishing personal networks.
- Protects the credibility of the Rawhide mission by complying with the Rawhide standard of conduct; setting an example, in words and actions, that is consistent with the values and beliefs of Rawhide.
- Contributes to team effort by performing other duties as directed or assigned by supervisor.

## Qualifications
**Job Qualifications:**

- Either a bachelor’s degree in a social or behavioral science field with two years of related work experience, or a master’s degree in a social or behavioral science field
- Strong interpersonal, organizational and administrative skills
- Excellent written and verbal communication skills
