**Position Overview**

The **Licensed Practical Nurse (LPN)** plays a crucial role in ensuring high-quality healthcare services for individuals of the Day Habilitation Program. Under the direction of the **Health Care Supervisor,** the LPN provides direct nursing care, and coordination of medical services, training, and supervision to the Individuals served. This role requires strong clinical skills, leadership abilities, and a commitment to promoting resident well-being and independence.

**Essential Responsibilities**

**Quality Assurance**

  • **Advise the Program Director and other DH staff of any medical problems that may hinder a member’s participation in DH or in a specific activity.**
  • Enforce **medical/nursing policies and procedures** to ensure consistent application across Day Habilitation Program.
  • **Administer medications**, as directed by prescriptions, standing orders, etc. Follow all regulatory requirements re: medication administration, security and storage.
  • Conduct **on-site health assessments and treatments** as needed.
  • Assist Developmental Specialists in securing necessary referral documentation and **program authorizations** for each recipient, and in maintaining currency of records on an ongoing basis.
  • Assist in maintaining **medical records, medical recommendations/authorizations and medication information for each individual,** and ensure that records/documentation meet regulatory requirements.
  • Ensure **compliance with best practices, state, and federal requirements**.

**Nursing Consultation, Case Management & Coordination**

  • **Complete nursing assessments under the direction of the Healthcare Supervisor.** Participate in Day Habilitation Service Plans and other team meetings, as requested. Effectively communicate with state agency personnel, other service professionals, family members, guardians, etc.
  • **Complete all nursing documentation and quarterly nursing notes, or more often if the member’s condition requires more frequent monitoring**
  • Assist in updating monthly progress notes and quarterly reviews and providing annual nursing assessments for individuals’ DHSPs.
  • Serve as a resource and advocate for health care access, ensuring individuals receive appropriate medical care.
  • Develop and maintain relationships with healthcare providers to facilitate quality care. **Coordinate medical services with each member’s PCP or medical clinic.**
  • **Obtain reports and approval of medical care plans from PCPs**

**Health Care Training & Compliance**

  • Stay current with **MassHealth and DPH regulations** to maintain compliance.
  • **Provide direct care and training in relevant areas.** Conduct annual and ongoing training **on person-centered medical guidelines, specialized training on emergency medical interventions** (ex: seizure protocols, epi-pens), **modified diets, vital signs, infection control practices** and new diagnoses as they arise.
  • Collaborate with **other CRC departments** to ensure individuals’ health, safety, and well-being.

**Additional Responsibilities**

  • Provide **emergency first aid** and accompany individuals to **emergency medical services** as needed.
  • Assist program staff in providing **direct care/personal care supports** to individuals

## Qualifications

  • **Must be licensed** by the **Massachusetts Board of Registration in Nursing** to practice as an LPN in the Commonwealth.
  • **Experience:** Minimum **one years of nursing experience** in a healthcare facility, school, or community setting.
  • Preference for experience with **developmental disabilities, nursing homes, or DDS/DPH regulations**.
  • **Legal & Compliance:** Must authorize **Criminal History Background Check (CORI)** and release information from **DPPC, DDS, and other regulatory agencies**.
  • **Training:**
  • **Successful completion** of all state and agency-mandated training and certifications, as assigned by CRC
  • **CPR/AED, first aid**
  • **Physical Ability:** Capable of performing essential job duties, with reasonable accommodations as needed.
  • **Driver’s License & Transportation:** Valid **Massachusetts driver’s license** and access to reliable transportation.
  • **Technical Skills:** Proficient in **Microsoft Office applications** and electronic health records (EHR).
  • **Understanding of behavior modification techniques.**

  • **Regular activities include:** sitting, standing, walking, stooping, kneeling, and crouching.

  • **Must be able to generate force to maneuver up to 100 pounds** (assistance provided when needed).
  • **Ability to lift up to:**
  • **30 pounds from floor to waist**
  • **10-15 pounds from waist to overhead**
