• **Pay Scale:**
• LPN: $24-$28/hr.
• **Summary:**
• Provides nursing services to the Facility Based Crisis centers

**Essential Duties and Responsibilities**

• Provides medication administration, physical health assessment/ screening, communicates with MD/ NP/PA for orders, obtains samples, may provide individual, group and family education related to medications or physical health processes and subsequent paperwork.
• Participates in interdisciplinary treatment planning, case support activities
• Provides crisis intervention consultation to all members of the team
• Participates in peer review process
• All other duties as assigned by supervisor

**Supervisory Responsibilities:** May provide supervision as directed.

**Education and/or Experience:**

**LPN:** Graduation from an accredited school of nursing and prefer one year of experience in nursing; or equivalent combination of education and experience.
