We are seeking a **Job Coach**to join our team of caring professionals.

### The **Job Coach** reports directly to the Community Services Program Coordinator. Responsible for assisting individuals in obtaining and maintaining competitive employment and for providing direct support and assistance to the people we support.

### RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO

  • Evaluates individual’s skills, aptitudes, and vocational potential to assist the individuals with attaining employment.
  • Assist in the planning and/or review of SEMP Staff Action Plan outcomes along with family, participant(s), family advisor and program coordinator.
  • Strong communication. organizational skills and experience in employment planning
  • Provide onsite training and specific interventions in order to help the individuals maintain job placement in the community
  • Advocate and support individuals with all employment goals.
  • Provide follow-up services according to Agency policy.
  • Actively communicate all relevant information to the Program Coordinator regarding individual related services.
  • Provide all interventions necessary of the individual to maintain their job status.
  • Write comprehensive daily and monthly progress notes on the agency EHR (eVero).
  • Comply with all standards for EVV-Electronic Visit Verification and agency standards for Paycom time and attendance Geo Fencing.
  • Attend all mandatory annual training and keep and maintain necessary training hours through Eleversity, Relias and other electronic platforms and portals as assigned.
  • Communicate with managers, employers, and potential business professionals in relation to individual employment standards.
  • All other duties necessary to provide continuous services to individuals.

## Qualifications

  • High School Diploma or equivalent required. Bachelor's degree preferred.
  • At least one year of related experience.
  • Ability to interact with families and community partners (i.e. employers and businesses).
  • Ability to always maintain professional appearance and demeanor.
  • Excellent verbal and written communication skills.
  • Computer literate and willing to utilize new electronic systems.
