Creekside Care Center of Gahanna is seeking a **motivated and compassionate Full-Time Physical Therapist Assistant (PTA)** to join our dynamic rehabilitation team! If you are passionate about helping patients regain mobility, independence, and quality of life, we would love to meet you.

Our therapy team works in a **supportive Skilled Nursing and Long-Term Care environment**, using **Net Health** documentation and collaborating closely with interdisciplinary staff to deliver exceptional patient-centered care.

### Responsibilities

• Provide physical therapy treatments under the direction of a licensed Physical Therapist
• Assist in implementing individualized treatment plans to improve mobility, strength, and function
• Document patient progress and treatments accurately using **Net Health**
• Communicate effectively with therapists, nursing staff, and interdisciplinary team members
• Educate patients and caregivers on therapeutic exercises and mobility techniques
• Maintain compliance with facility policies and regulatory standards

## Qualifications
### Qualifications

• Graduate of an accredited **Physical Therapist Assistant (PTA) program**
• Current PTA license in the state of Ohio (or eligible for licensure)
• Experience in **Skilled Nursing Facility (SNF) or Long-Term Care (LTC)** preferred, but not required
• Strong communication and teamwork skills
• Passion for improving the lives of patients and residents
