**Pay Range $53.71 - $60.00 Per Hour (Depending on Years of Experience)**

**Centurion is proud to be the provider of comprehensive healthcare services to the Maryland Department of Public Safety and Correctional Services.** We are currently seeking a **Full-Time RN Discharge Planner**to join our team at **BCBIC (Baltimore Central Booking & Intake Center)**located in **Baltimore, Maryland.**

The **RN Discharge** **Planner**is responsible for coordinating patient discharge planning from a correctional setting. The Discharge Planner will work under the direction of the Director of Nursing in collaboration with a multidisciplinary team to develop and implement a safe and appropriate discharge plan to promote continuity of care after release.

## Qualifications

**QUALIFICATIONS**

• Current Registered Nurse in the state of Maryland or the ability to obtain license required
• Current BLS and CPR certifications required
• On-year experience as a Discharge Planner or Case Manager required
• Experience in corrections or an acute setting preferred
• Ability to obtain a security clearance to include drug screen and criminal background check
