| Job Purpose | Provide direct support to individuals with disabilities. |
| Job Duties & Responsibilities | Demonstrate CORE4 values in the execution of mission and vision. Complete documentation of services provided. Demonstrate comprehension of abuse, neglect, and exploitation and report accordingly. Ensure the health, safety, and welfare of participants always. Maintain a clean and safe work environment. Accurately report time worked and appropriately use payroll software. See the addendum for department-specific job duties and responsibilities. Other duties as assigned |
| Required Qualifications | Capacity to demonstrate CORE4 values. High School Diploma or equivalent. Experience in the field of disabilities is preferred. See the addendum for department-specific qualifications |
| Working Conditions | With limited exceptions, Monday – Friday Regular business hours See the addendum for department-specific working conditions |

Other duties as assigned. - Assist in production and quality standards. Intercept for quality control. Assist with the work of team members and staff. - Assist with the set up and break down work area each day. Communicate effectively with staff and team members. - Aid team members as needed, such as personal care and medications. - May attend meetings. - Complete daily notes and monthly summaries as required. - Maintain all Medicaid and Zoom Group documentation and training requirements. - Complete all tasks according to the tasks analysis related to the assigned logistics account. |

| Working Conditions | The work environments are warehouses, and manufacturing locations. Environments may be climate and non-climate controlled. Warehouses and manufacturing may be loud, dusty with moving equipment, such as forklifts, conveyors, and roller lines. |
