Join AmeriCorps as a Health Educator with a focus in Health Promotions at Erie Family Health Centers. This full-time service year opportunity supports health education, chronic disease prevention, patient engagement, and HIV-focused outreach through the Lending Hands for Life program. Members gain hands-on experience in community health while making a meaningful impact on underserved communities.

This is a year-long, full-time AmeriCorps position requiring completion of a minimum of 1,700 service hours. The work schedule for this position is aligned with Erie's hours of operations.
**Main Duties & Responsibilities:**
- Provide one-on-one health education and self-management support to patients
- Participate in community events, training, and AmeriCorps service activities
- Support Lending Hands for Life HIV programming and re-engagement efforts
- Conduct outreach and engagement for patients requiring additional support
- Document patient interactions and coordinate follow-up care activities
- Facilitate health education groups and recruit participants for programs

## Qualifications
**Education**
**Required**
- High school diploma or equivalent.
- **Skills and Knowledge**
- **Required:**
- Strong verbal and written communication skills.
- Ability to work with diverse populations.
- Current CPR Certification.
- U.S. citizen, national, or lawful permanent resident.
- Ability to meet AmeriCorps background check requirements.
**Preferred:**
- Experience in community health, social services, or related fields.
- Intermediate Microsoft Office skills.
- Experience with electronic health record systems.
- Bilingual English/Spanish skills.
