# **Community Care Navigator Healthy Alliance Program**

**Full-Time | Monday-Friday | No Weekends**

Community Wellness Partners is seeking a compassionate, organized Community Care Navigator to join our Healthy Alliance team. In this role, you'll help individuals access healthcare, food, housing, transportation, and other community resources that support their overall health and well-being.

## **What You'll Do**

  • Manage incoming referrals and provide timely outreach and follow-up.
  • Complete screenings and eligibility assessments.
  • Connect individuals with health and community resources that address health-related social needs.
  • Coordinate referrals and help individuals navigate available services.
  • Maintain accurate documentation in electronic systems.
  • Collaborate with healthcare providers, community organizations, and internal team members.
  • Monitor referral outcomes and assist with resolving barriers to services.
  • Track data and identify opportunities to improve the program.
  • Participate in community outreach activities, meetings, and events.
  • Stay current on programs, community resources, and policies that impact the people we serve.
  • Perform other duties as assigned.

## Qualifications

## **Education & Experience**

  • High school diploma or GED required.
  • Associate degree in health, human services, social work, or a related field preferred.
  • Experience in healthcare, social services, customer service, case management, or community outreach is preferred but not required. Equivalent education and experience will be considered.
