**Job Summary**We are seeking a skilled **Certified Occupational Therapist Assistant (COTA)** to join our team at *Careview Transitional Care of Weston*. **Responsibilities**

• Evaluate patients' needs and develop personalized treatment plans
• Implement therapeutic interventions to enhance patients' functional abilities
• Monitor patient progress and adjust treatment plans as necessary
• Collaborate with interdisciplinary teams to ensure comprehensive patient care
• Educate patients and families about treatment goals and home exercises
• Maintain accurate documentation of patient progress and treatment outcomes

**Requirements**

• Current state licensure as a COTA
• Strong knowledge of occupational health practices and rehabilitation techniques
• Excellent communication skills for effective interaction with patients and team members
• Experience in acute care or skilled nursing facilities preferred
