**Job Summary**

*Careview Transitional Care of Weston* is seeking a dedicated and compassionate **Physical Therapist Assistant (PTA)** to join their team. In this role, you will work closely with licensed physical therapists to implement treatment plans for patients recovering from injuries or managing chronic conditions. You will play a crucial role in enhancing patient mobility, alleviating pain, and promoting overall wellness in our residents. The ideal candidate will have a strong understanding of medical terminology, anatomy, and various therapeutic techniques. **Responsibilities**

• Assist physical therapists in developing and implementing individualized treatment plans for patients
• Provide direct patient care, including administering therapeutic exercises and modalities
• Monitor patient progress and report changes to the supervising therapist
• Educate patients and their families on treatment protocols and home exercise programs
• Maintain accurate documentation of patient treatments and progress notes
• Participate in discharge planning to ensure continuity of care
• Collaborate with healthcare professionals to optimize patient outcomes
• Utilize knowledge of trigger point therapy, pediatrics, geriatrics, and other specialized techniques as needed

**Requirements**

• Associate degree from an accredited Physical Therapist Assistant program
• Current state licensure or eligibility for licensure as a PTA
• Strong knowledge of medical terminology and anatomy
• Experience in outpatient settings preferred; acute care experience is a plus
• Familiarity with discharge planning processes
• Excellent communication skills and ability to work effectively within a team
• Compassionate approach to patient care with a commitment to improving patient quality of life
• Previous experience with trigger point therapy is advantageous but not mandatory
