## Description
**Location:** Framingham, MA - Outpatient Treatment Program
**Schedule:**16 hours per week. Thursday and Friday (days may change depending on needs of the program)
**Pay rate:** $22.90/hour

The **Recovery Support Navigator** is responsible for

• Providing quality services and case management to assigned patients in accordance with the Recovery Navigator model.
• Performing intake assessments on all new patients to the program and completes all associated documentation within required time frames.
• Working with the patient to develop and implementing an individualized plan with goals and objectives based on identified needs.
• Assisting patients with scheduling and coordinating medical and behavioral health appointments.
• Coordinating care and the plan with existing providers including but not limited to behavioral health, primary care and Community Partner.

## Qualifications
**Qualifications:**

• Bachelor’s Degree in human services related field or discipline
• Or LADC
• Or has a recognized certification and/or licensure for the target population to work with
• Orsufficient experience and training in counseling and meets regulatory requirements
