Youth Haven Services is seeking a compassionate and dedicated **Qualified Professional (QP)** to provide **Intensive In-Home (IIH)** services to children, adolescents, and their families throughout **Person County, NC**.
This position is ideal for professionals who enjoy working independently, building meaningful relationships with families, and helping youth remain safely in their homes and communities. As a Qualified Professional, you will provide clinical oversight, case management, and skill-building services while working closely with licensed therapists and community partners to help families achieve lasting success.

## Position Summary
The Qualified Professional provides Intensive In-Home case management and rehabilitative services for children and adolescents with behavioral and mental health needs. Services are delivered primarily in the client's home and community and are guided by each individual's Person-Centered Plan. This position works collaboratively with the licensed therapist, family members, schools, and other providers to ensure coordinated, quality care.
**This is a contract position and compensation is based on billable client contacts.**
## Essential Responsibilities
- Complete client intakes, assessments, and Person-Centered Plans (PCPs).
- Submit initial authorizations and reauthorizations for Intensive In-Home services.
- Coordinate care with therapists, families, schools, physicians, and other community providers.
- Meet with clients and families in their homes, schools, and community settings to provide case management and skill-building services.
- Monitor client progress and assist families in achieving treatment goals identified in the Person-Centered Plan.
- Maintain accurate, timely, and compliant clinical documentation, including progress notes, service logs, treatment updates, and required agency paperwork.
- Complete NC TOPPS assessments, Service Authorization Requests (SARs), discharge documentation, and other state-required documentation.
- Maintain assigned caseloads and ensure authorizations remain current.
- Assist families in accessing community resources and support services.
- Participate in Child and Family Team (CFT) meetings and other treatment planning meetings as needed.
- Perform other duties as assigned in support of the Intensive In-Home program.
## Qualifications
### Education
- Bachelor's degree in a Human Services-related field is required.
### Experience
- Minimum of two (2) years of documented experience working with children, adolescents, and families receiving behavioral health or mental health services.
### Additional Requirements
- Valid North Carolina driver's license.
- Reliable transportation with current automobile insurance.
- Ability to travel throughout Person County to provide services in clients' homes and community settings.
- Ability to maintain a flexible schedule, including afternoon and evening appointments based on family needs.

- Sound clinical judgment and effective problem-solving skills.
- Proficiency with electronic health records and Microsoft Office applications.
- Professionalism, flexibility, and a commitment to providing family-centered care.
## Compensation
**This position is compensated on a per-contact (billable service) basis.** Qualified Professionals have the flexibility to manage their schedules while meeting the needs of assigned families and maintaining productivity expectations.
