## What You'll Do

As a Care Coordinator, you'll play a key role in scheduling and coordinating patient care by

  • Responding promptly and professionally to pharmacy inquiries regarding patient services.
  • Coordinating patient schedules by matching the most qualified nurse based on location, therapy, and patient-specific needs.
  • Ensuring all required documentation is received and provided to our nurses before care begins.
  • Verifying assigned nurses have the necessary training and competencies for each patient's therapy, coordinating additional training when needed.
  • Communicating with nursing staff regarding schedules, patient care plans, and required documentation.
  • Serving as the primary liaison between patients, nurses, pharmacies, and physicians to ensure seamless communication and outstanding patient support.
  • Utilizing agency software and technology to document patient visits, coverage, special requirements, laboratory information, and order expirations.
  • Maintaining accurate, timely documentation of all patient communications, concerns, and updates.
  • Participating in team meetings focused on process improvement, patient growth, and operational excellence.
  • Maintaining strict patient confidentiality while adhering to all HIPAA and ethical standards.
  • Reporting patient care concerns promptly to clinical leadership.
  • Keeping nursing staff informed of any changes to patients' plans of care.

## Qualifications
**Required:**

  • High school diploma or GED.
  • Reliable transportation.
  • Strong computer and organizational skills.
  • Excellent verbal and written communication skills.

**Preferred:**

  • Associate's or Bachelor's degree.
  • Previous experience in healthcare scheduling, care coordination, medical office administration, home health, infusion services, or customer service.
