## Clinician Case Manager IHB

**POSITION SUMMARY:**

As part of an Intensive Home-based Service, under professional supervision, the Outreach/Homebased Clinical Therapist is responsible for the provision of therapeutic and case management services to children and families being served by the program.

**RESPONSIBILITIES AND DUTIES:**

• Conduct comprehensive biopsychosocial assessments (and psychological testing if M.A. or M.S. is in clinical psychology)
• Formulation of person-centered treatment plans
• Provide individual, family, and group psychotherapy
• Collaboration and participation with other clinical staff and treatment team members in planning and guiding treatment
• Assist families with accessing and utilizing community resources
• Liaison with natural supports, community and other agencies in working toward therapeutic goals

## Qualifications
**MINIMUM JOB REQUIREMENTS:**

• Master’s degree in Social Work, Psychology, Counseling, Guidance or other mental health discipline from an accredited school
• Has one year experience and training in examination, evaluation and treatment of minors and their families
• Appropriate professional registration/certification by State/national authority required (L.M.S.W., L.L.P., R.N., etc.)
• Possession of a valid driver’s license and have access to a private vehicle for day-to-day job performance
