**HEALTH CARE COORDINATOR**

**Monday-Friday 6:30am-2:30pm Full Time 40 hour position**

Provide direct care supports to this all male home for people between the ages of 20-70 with developmental disabilities in this residential home in Hyde Park, NY. This position serves as an assistant to the RN and Residence Manager – makes doctor’s appointments, administers medication, tracks heath data, and more. Candidates must have current AMAP certification or ability to achieve AMAP certification within 3 - 6 months. We will train you.
