**Job Title:** Medical Coordinator
**Employer**: Chesterwye Center, Inc.
**Department**: Quality Enhancement
**Reports to**: Director of Quality Enhancement
**FLSA Status:** Non-Exempt
**Pay Range:** $21 to $23
**Classification:** Full Time, Monday – Friday, Some Weekends (Once Per Quarter)
**Effective Date**: 12/10/2024; Updated: 06/26/2026
**Job Responsibilities:**

  • Coordinate healthcare appointments for clients, managing and logging details

regarding missed, canceled, or rescheduled appointments.

  • Facilitate communication between healthcare providers and nursing staff.
  • Manage the 90-day PMOF log and ensure physician signatures are obtained before

expiration.

  • Oversee medical record compliance, including file management and updates to

Primary Care Provider (PCP) records.

  • Give medical updates to the operation staff prior to the individuals' PCP meetings.
  • Attend PCP meetings as required.
  • Handle distribution and logging of medications, MARs, PMOFs, and medications to

Residential staff.

  • Facilitating distribution of medications to Residential staff, medication reconciliation,

and logging on eMAR.

  • Support the RN by reviewing eMARs, the electronic medical record system, and

Medication Binders.

  • Maintain organization and compliance of individual binders in the Medical Room.
  • Deliver basic first aid and medication administration in the Medical Office as needed.
  • Perform clerical duties to support compliance with OHCQ/DDA licensing and MBON

requirements.

  • Manage inventory and order medical supplies for Medical Offices and Residential

homes.

  • Handle pharmacy orders and verify deliveries.
  • Prepare and distribute communications from the Medical Services department.
  • Assist/ support with transporting individuals to healthcare appointments as required,

ensuring safe use of mobility aids.

  • Train Direct Support Professionals (DSPs) in the use of Assistive Technology.
  • Advocate for client rights and dignity.
  • Adjust work hours as necessary to meet the health care needs of the people supported.
  • Provide direct support to ensure adequate care and staffing ratios.
  • Perform additional duties as directed by the Supervisor

## Qualifications
**Job Requirements:**

  • High School Diploma or GED
  • Valid Maryland Driver's License with a clean driving record
  • Working knowledge and understanding of Microsoft Office.
  • Certifications required: Mandt, CPR/First Aid, CMT, etc. The certification of these three must

be completed within the first 90 days of hire.

  • Compliance with all DDA & Chesterwye required trainings.
  • Flexible schedule during work hours
  • Ability to lift up to 50 lbs and push/pull up to 100 lbs
  • Attention to detail.
